Our team is dedicated to providing high-quality, person-centered services that empower each individual to live a meaningful, engaged, and self-directed life. We offer a full continuum of supports designed to promote independence, connection, and well-being.
Our day training program supports adults in building skills, friendships, and purpose. We go beyond coloring pages and TV time. Our programming is creative, individualized, and centered around connection.

Family Home Provider Program (FHP)
We work with incredible families who open their homes to adults with disabilities. Our FHP program matches clients with carefully selected caregivers to create safe, stable, and nurturing home environments.

Behavior Support Services
We provide individualized, compassionate guidance for children and adults receiving supports through the Michelle P. and SCL waivers.
Focusing on understanding each person’s unique needs, strengths, and communication styles, we create effective, person-centered behavior plans. Through ongoing assessment, skill-building, and supportive coaching, we help individuals develop meaningful coping strategies, enhance daily functioning, and improve overall quality of life.
Community Living Supports (CLS)
We provide one-on-one, person-centered assistance designed to help individuals engage in their community with confidence and purpose.
We help individuals pursue their goals, build daily living skills, and participate in meaningful activities. Whether it’s enjoying a meal out, cashing a check, planning and shopping for weekly groceries, tidying their home, or exploring local opportunities, CLS empowers each person to live more independently, stay connected, and thrive within their community.
Person Centered Coaching
Person Centered Coaching is an SCL-only service designed to help individuals and their support teams successfully carry out the goals and strategies outlined in their person-centered plan of care. When barriers arise that make implementation difficult, a PCC provides targeted, individualized guidance to keep the plan on track. Working closely with the participant, their team, and often under the direction of a Positive Behavior Support Specialist, the coach helps monitor progress, reinforce strategies, and problem-solve challenges. PCC ensures that supports remain consistent, effective, and aligned with the participant’s goals—empowering them to move forward with clarity and confidence.
Respite
Respite services, offered through the Michelle P. waiver, provide essential relief and support to an individual’s primary 24/7 caregiver. This service ensures that caregivers can take time to rest, attend to personal needs, or manage responsibilities while knowing their loved one is safe, supported, and engaged. Respite promotes caregiver well-being and stability, while also offering individuals quality care from trained staff in a comfortable, person-centered environment.
